In the end, for whatever reason, the resources that exist werent able to help Shane.

No, these programs and resources arent end-all-be-all fixes, and they dont ensure #Insulin4all. But they can help in an emergency for those facing a dire life or death choice.

Something more has to be done, without waiting for sweeping fixes in drug pricing structures and healthcare policy. NO ONE should be dying because they couldnt obtain a single vial of insulin, which is not in short supply in this country.

Here at the Mine, were big proponents of crowdsourcing innovative ideas so here is possibly the most important challenge ever put to our community:

What else can we do, at the local and grassroots level, to help people like Shane and Kevin and so many others who are falling through the cracks?

Ways You May Be Sabotaging Your Insulin Therapy

According to the Centers for Disease Control and Prevention, DKA hospitalization rates for people with diabetes rose 54.9 percent from 2009 to 2014, reversing a prior trend of decline, though in-hospital fatality rates continued to fall through 2014. The agency called for further study to identify the causes.

Hyperglycemic hyperosmolar nonketotic syndrome is another condition that occurs when insulin deficiency causes blood sugars to exceed 600 mg/dl, notes Summit Medical Group. The body responds by excreting the extra blood glucose in the urine, leading to severe dehydration and an imbalance of electrolytes. Brain swelling, seizures, organ failure, coma, and even death can occur.

Miller says he has seen his patients admitted to the ICU due to hyperglycemic crises brought on by insulin rationing. Thank God I have not had a patient die yet, and I say yet because if the crisis continues, its a matter of time.

Get Prescription Insulin Using Cost Reduction Routes

You can, of course, get prescription insulin by paying the full cost if you dont have insurance. This may be the most effective option if you can afford your prescribed insulin without the help of insurance.

You dont need to have insurance to get a prescription, as many misunderstand. You can visit a doctor and get a prescription without any insurance coverage.

Paying the full out-of-pocket cost for the prescribed insulin means that you do not have to worry about the effectiveness of over-the-counter insulin. This is because you know that youre taking the right insulin at the end of the day, which suits your condition and not a cheap alternative.

However, insulin is still expensive, and getting the most effective insulin without insurance can be way too expensive for many. So, you can check for the following options, which can, to some extent, reduce your full cost.

    • Opt for generics and biosimilar insulins, which are low cost comparatively.
    • Vials can be cheaper than pens
    • Check for older versions of the brand-name insulin

For all this, you will need to request that your doctor prescribe one of these options after discussing your financial condition and if they can give the same effect as the insulins prescribed.

Some Cgm Sensors Cost Less Without Insurance

If you dont have health insurance, this doesnt always mean you cant access a continuous glucose monitor. While health insurance may make it easier for some people with diabetes to access a CGM at an affordable rate, this isnt always the case.

Even if you have a high deductible healthcare plan , you may find that some continuous glucose monitors like the Freestyle Libre 2 cost less out of pocket without running insurance.

A Freestyle Libre 2 sensor can be purchased without insurance at many pharmacy counters across the United States for typically around $130-140 for two sensorsabout a months worth of supplies. So long as you have a prescription for the product, you should be able to fill it and purchase it for around this price point.

How Much Is Insulin With Insurance

If you do have insurance, the bill can still be significant, given the copays and cost of supplies. A few things that can affect the cost of insulin with insurance include whether youve met your deductible, which tier your insulin is on in your insurance plans drug formulary, how much insulin you need, and which pharmacy you fill your prescription at.

FDA approves first-ever generic version of insulin, which will dramatically cut costs

In most circumstances, it is illegal for individuals to import FDA-approved drugs from other countries for personal use. However, based on changes enacted by the MMA, personal importation of prescription drugs that have not been approved by the FDA for use in the U.S. is permitted on a case-by-case basis.

    Saving Money On Cgm Readers

Abbott lists the price of the Freestyle Libre 2 handheld reader at $65 when run with insurance. But the Freestyle Libre 2 reader can also be replaced by its corresponding smartphone app, which has the same features, benefits, and functionality. So, if you dont have insurance and already have a smartphone, this could be another way to save money and ensure access to all the technology you need.

If you dont have a smartphone or insurance, various websites list the Freestyle Libre 2 reader for anywhere between $100 to $150. While this can be a significant expense upfront, it should only be a one-time expense that lasts many years, saving you money over time.

Can I Apply To More Than One Pap

    • Because PAPs are organized by manufacturer and not specific drugs, you can get assistance for multiple drugs from the same manufacturer without filling out multiple applications.

    • Being enrolled in one manufacturers PAP does not prevent you from applying to a different manufacturers PAP, as long as you also have a prescription for that companys drug.

    • A general rule of thumb is that you cannot use a PAP for a drug while accepting another form of assistance or government reimbursement for the same drug.

How Long Does A Vial Of Insulin Last

    Freestyle Libre 14 Day

Opened vials, whether or not refrigerated, must be used within 28 days. They must be discarded if not used within 28 days. If refrigeration is not possible, the open vial in use can be kept unrefrigerated for up to 28 days in a place away from direct heat and light, as long as the temperature is not > 86°F .

Other Private Insurance Options For Individuals And Families

In most states, a person may choose to buy health insurance directly from an insurance company outside the Marketplace. Plans sold outside the Marketplaces are also available during the open enrollment periods for the Marketplace , but you will not be able to get the financial help paying for your insurance – financial help is only available through the Marketplaces.

Starting in 2014, all new individual health insurance plans, whether sold inside or outside the Marketplace, cannot deny coverage, charge more, or refuse to cover treatments because you or someone in your family has diabetes.

Due to a federal announcement in 2014, some people may be able to continue to reenroll for a few more years in the individual health insurance policy they had before 2014. However, the insurance policy will not provide all of the rights and protections that new individual policies sold inside and outside the Marketplace must follow starting in 2014.

When shopping for a health plan, it is important to check if the plan covers the diabetes supplies, services, and prescription drugs you need, and what it costs. If you want to keep your current health care providers, check if they participate in the plan.

    Why Insulin Remains Unaffordable

But why does insulin a medication thats been around for more than 100 years remain unaffordable for many people in the U.S.?

The high cost can be attributed in part to evergreening, a process in which drug companies make incremental improvements to their products that can extend the life of their patents, said Dr. Kevin Riggs, a physician at the University of Alabama at Birmingham Heersink School of Medicine. He co-wrote a study published in the New England Journal of Medicine in 2015 that described the century-long history of the drug.

The improvements may include tinkering with a molecule or changing the delivery system, such as using insulin pens instead of vials.

Extending patentscan discouragegeneric drugs from being developed, Riggs said, allowing drugmakers with exclusive rights to their insulin to charge whatever the market will bear. And as supply chains have become more complicated over the years, costs have ballooned.

And so that means those prices have gone up crazy, he said.

And even when the patents do expire as many have Riggs said that the large investment it takes to get insulin manufactured and approved by U.S. regulators may make the venture less appealing to generic drugmakers.

Eric Tichy, division chair of pharmacy supply solutions for the Mayo Clinic in Rochester, Minnesota, agreed, saying the barrier to entry to produce insulin is pretty high.

No one should be “forced to make the decision between life or death,” she said.

    Corporate Human Rights Responsibilities

Even in the absence of regulations, under the UN Guiding Principles on Business and Human Rights , all businesses have a responsibility to respect human rights and ensure that they do not cause or contribute to actual or potential adverse human rights impacts.

Fundamental to this responsibility is the requirement that companies carry out human rights due diligence to identify the possible and actual human rights impacts of their operations, and to establish meaningful processes to prevent and mitigate those risks and remedy adverse impacts. The process should be ongoing and continuous.

Companies should meaningfully consult with potentially affected groups and other relevant stakeholders at all stages of human rights due diligence. This consultation should be meaningful and go beyond engaging with a few patient representatives.

Human rights due diligence also entails identifying and rectifying business practices with adverse human rights impacts that are part of a companys operations. The UN Guiding Principles state that businesses should look at their own activities and embed human rights policies across all relevant business functions.

Pharmaceutical companies also have responsibilities as outlined in the 2008 Human Rights Guidelines for Pharmaceutical Companies in relation to Access to Medicines , issued by the UN Special Rapporteur on the Right to Health.

Drug Price Rebates For Pbms: Can The System Be Fixed

In January 2019, the Department of Health and Human Services Office of Inspector General proposed a rule that would lift regulatory protection for rebates on prescription drugs paid by drug makers to PBMs, Medicare Part D plans, and Medicaid managed care organizations. The rule would do this by removing an existing provision of the Anti-Kickback Statute that protects rebates to PBMs. Advocates, including Heilaman, hope this step would eliminate kickback activity in the drug pricing and rebate system. The rule also would create two new protections: for discounts offered to patients at the pharmacy , and for fixed-fee services deals between manufacturers and PBMs.

Every day, Americans particularly our seniors pay more than they need to for their prescription drugs because of a hidden system of kickbacks to middlemen, said the Health and Human Services secretary, Alex Azar, in a . President Trump is proposing to end this era of backdoor deals in the drug industry, bring real transparency to drug markets, and deliver savings directly to patients when they walk into the pharmacy.

JDRF, a research and advocacy organization for people with type 1 diabetes, is pleased HHS has proposed ending the use of rebates in federally funded healthcare programs, said Cynthia Rice, the senior vice president of advocacy and policy, in an email statement.
